• Pay a Bill. To pay a bill online, you will provide us with your account number, name, and date of birth.
• Request Submission Form. To submit a Patient Request to Access Medical Records or Authorization for Use and Disclosure form, you will provide your name, date of birth, address, phone number, and signature.
• Contact Us. To contact us, you will provide your name, email address, phone number, address, and any other information you choose to provide.
• Your search queries on the Site.
• Other information you voluntarily provide to opt in to receive future communications from us.

Universal Opt-Out Mechanisms

We do not engage in “sales” or “targeted advertising” as those terms are defined under applicable laws, and therefore our Site does not recognize the Global Privacy Control (“GPC”) signal.
